Defining Supported Living At Anchits

Supported living is service that allows people to access physical and emotional support with trained staff while living within their home that enables them to live a fulfilled independent life. We understand that living by yourself and away from family can be overwhelming therefore we ensure you have time to become familiar with the surroundings of your new home and meet the staff who will be readily available to support you and show you around the facility. We ensure you have a healthy balance in your lifestyle supported by encouraging, positive community we ensure your transition into your new space is fulfilling.

Process

Our Supported Living Processes

At Anchits we will take time to meet you first and do a needs assessment, as well as understand your present, past and future needs. This enables us to provide the best care and support. We conduct meetings and interviews to get as much information from yourself, your family, relatives, and any health professionals you have worked with such that your best wishes for the future are met and our team will then create a person-centred support plan that will be reviewed regularly to suit any changes you may have.
Things to consider before moving into Supported Living:
  1. Ensure you have contacted your local council and GP so that your funding for the service can be provided as well as the allocation of your Case worker /social worker who will support you and ensure all your needs are met.
  2. Once you have approval for your funding, you can now contact our Management at info@anchits.com or use the Referral form to send us your inquiry.
  3. Our team will respond and contact you to set an appointment to discuss your requirements and do a needs assessment.
  4. We will invite you to come and view the property and you can choose which facility best meets your needs.
  5. We will then provide a proposal for your funding requirements that you will then forward to your council for approval.
  6. We will then provide you with a move-in date and support with the transition to your new space.
